Do you know an exceptionally brave person who deserves special recognition?

If he or she is chosen as ABC News’ winner, your nominee and one guest will win a trip to New York City to see Jennifer Lopez live in concert on ABC-TV’s “Good Morning America” on Oct. 9, 2007, performing selections from her new album, “Brave.”

Fill out the information form, including a short essay no more than 300 words telling us about a brave person. You may include a photo or video, but it’s not necessary to win.
